What a blank bill of sale is for
A bill of sale is the receipt for a private sale between two people: who sold what, to whom, on what date, and for how much. A blank one is simply that form before anyone fills it in.
It matters most when the thing being sold has no title of its own. For a tractor, an ATV, a golf cart or a set of tools, the signed bill of sale is the ownership record. For titled property like a car or a boat, it sits alongside the title as proof of the price and the date.

What goes on a blank bill of sale
Names, addresses and phone numbers for both parties. A description of what is being sold, with whatever number identifies it: a VIN, a hull ID, a serial number, an IMEI. The price, how it was paid, and anything still owed. A statement that it is sold as is, with a line for known defects. A seller's certification that it is theirs to sell and free of liens. Then signatures and dates.
Leave nothing blank at signing. Write N/A or draw a line through anything that does not apply, so nothing can be added afterwards.

Is a handwritten bill of sale valid?
Generally yes, as long as it identifies both parties, describes what was sold, states the price and date, and is signed. A printed form is simply harder to dispute later, and less likely to leave out something your state asks for.
Do I need to notarize a blank bill of sale?
Notarization happens after it is filled in, not before, and most states do not require it for a private sale. Page 2 of every form here is a notary acknowledgment for the states and situations that do.
Can I use one bill of sale for two items?
You can if they are sold together in one transaction. Use the general form and describe both items with their serial numbers, or the livestock form if you are selling several animals at once.
Do both people sign a bill of sale?
Yes. Both sign, both date it, and both keep a copy. A form signed by only the seller is weaker evidence if the sale is ever questioned.
