Copyright and DMCA

Our templates are our own work. You may use them freely for your own sales; what you may not do is resell the blanks as your own.

What you may do

You may download, print, fill in and use our templates for your own transactions, personal or business, at no cost. You may give a completed copy to the other party to your sale, keep copies for your records, and print as many blanks as you need.

You may link to any page on this site freely.

What you may not do

You may not resell the blank templates, present them as your own work, or republish them in bulk on another site, app or service. The wording, layout and page content on this site are ours.

Government forms are not ours

Where we link to an official form published by a state, province or agency — for example California's REG 135, Florida's HSMV 82050 or ICBC's APV9T — that form belongs to the body that publishes it, and we link to the official source rather than hosting a copy.

Reporting copyright infringement

If you believe material on this site infringes your copyright, email contact@scurgemedia.com with the subject line "DMCA notice" and include: identification of the work you say is infringed; the exact address of the material on this site; your name, address, telephone number and email; a statement that you believe in good faith that the use is not authorised by the copyright owner, its agent or the law; a statement that the information in your notice is accurate and, under penalty of perjury, that you are the owner or authorised to act for the owner; and your physical or electronic signature.

We will review complete notices promptly and remove or disable material where the complaint is valid.

Counter-notice

If your material was removed and you believe that was a mistake or a misidentification, you may send a counter-notice to the same address, including the material and where it appeared, your contact details, a statement under penalty of perjury that you believe in good faith it was removed by mistake, and your consent to the jurisdiction of the appropriate court.
